Below are a couple forms that need to be filled out before your club season can start. All forms and paperwork need to be completed and brought with you on Commitment Night. 1. Fill out our Athlete Info Form. This is an online form used by ProLink for informational purposes only. Just fill it out online, click “Submit” and you’re done.
2. 2011 Player Medical Release – This is a medical release form that coaches are required to have with them at all times in case of emergency. You should download, print and bring the completed form with you on commitment night. Please note that this form must be notarized. Notaries can be found at most banks and some supermarkets.
3. Player/Parent Handbook 2010 – Download and review. This is an overview of policies, procedures, expectations and rules for the upcoming club season. No need to print it out, just familiarize yourself with the contents.
4. Player/Parent Agreement 2010 – This is an agreement between players, parents, and ProLink VBC stating that we have all read and understand the Player/Parent Handbook. Download, print, sign and turn in this agreement on commitment night.
Also due on commitment night is your initial club dues payment. Contact a director if you have any questions.
