Congratulations on your decision to play for ProLink Volleyball Club this season! We’re excited to get underway, but before we do, there are a few simple steps necessary to complete the commitment process.
- Download and Print the Financial Agreement Here. Because we want to make it as easy as possible to play club volleyball with us, we offer flexible payment plans. Please fill out the form with a payment plan that works for you. We’ll review and sign off when you come in.
- Be sure to fill out your team and the dues associated with that team
- Print your player’s name clearly
- Fill in the plan that works for you along with payment due dates
- Sign and initial where necessary
- Submit the initial club dues down payment. We accept cash and checks. We can also accept credit cards for a small 3% processing fee (Please contact us to use this option).
- Submit the SRVA Medical Release Form. This form will need to be printed, filled out completely with signatures, and notarized.
- Go to the SRVA website here: http://hai3.srva.org/pbl/R_SPLASH.C.php
- Click on “Registration for Individuals”.
- Enter your information to access your account (detailed instructions on how to do this are located on the SRVA page).
- Choose the option to Create and Print the Medical Release form.
- Fill out, sign, and notarize.
- Review the Player/Parent Handbook here. This outlines some of the procedures and policies we will observe during the season.
- Download and print the Player/Parent Agreement here. Fill out and sign where necessary. This simply acknowledges that you have read and understand the Handbook.
